Requirements:
Thank you for entrusting us with your son or daughter’s youth football or cheer experience. We take this responsibility seriously and will do our best to make sure everyone involved has a fun and memorable year.
This year we are proud to continue with the Watchung Hills Football League. A league designed to “Keep Football and Cheer Fun for Everyone.” We will also be maintaining our two travel teams that will be competing in the Junior Skyland Football Conference.
We are pleased that you have decided to register your son or daughter for these exciting programs and we are sure you will not be disappointed. Please read through this information carefully, it outlines important information about our program.
The following is a list of all the required documents which must be furnished or completed by the first day of practice.

Notarized Copy of Birth Certificate:
**New
players and cheerleaders only. A NOTARIZED copy BEARING A RAISED SEAL will be accepted. All notarized copies of birth certificates will NOT be returned.

Photocopy of Report Card:
or equivalent for ALL academic grading periods during the 2006-2007 school year.
For Biddy and Midget Players and Cheerleaders Only.

Medical Emergency Consent Form:
Must be filled in completely and NOTARIZED.

Athletic Pre-Participation Physical Part A:
This is a standard form issued by the state. Both sides must be completed by the child’s parent or legal guardian.

Athletic Pre-Participation Physical Part B:
This is a standard form issued by the state. Both sides must be completed by the child’s physician after January 1st of the current year.
Because of insurance liability issues no child will be allowed to start practice without a completed Medical Emergency Consent form, an Athletic Pre-Participation Physical Part A and an Athletic Pre-Participation Physical Part B form on file with the organization.
Forms will
be collected at Cheer Uniform Measurements, Family Fun
Day, Parents Meetings and first day of Practice ONLY.
